What is Person-Centred Care?
Person-centred care means support that’s shaped around the individual, not the service. At Fairmont, this begins by understanding each person through observation, input from families and professionals, and a detailed look at what helps them feel safe, settled, and valued.
We focus on everything from preferred communication styles and sensory needs to the structure of a person’s daily routine. Whether someone uses few or no words, needs visual prompts to feel secure, or benefits from consistent, predictable activities, we adapt around them, always providing a respectful, inclusive, and supportive environment.
Implementing the Care Model
Our care model is built on structured, personalised routines, using recognised approaches like the TEACCH framework and Positive Behaviour Support. Daily plans are shaped by each person’s preferences, goals, and sensory profile to ensure they feel safe, supported, and understood.
We use visual schedules, tailored activities, and communication supports such as PECS or Makaton. Staff are trained to recognise and respond to behaviours that communicate need, adapting their approach to suit each individual’s pace. Using Nourish Care, a digital care planning system, we record and reflect on daily interactions in real time — helping us continuously refine and personalise support.
Collaboration and Family Involvement
Families are central to everything we do. We believe no one knows a person better than those who’ve walked the journey with them, which is why we involve families in planning, reviews, and everyday conversations about care.
Our team provides regular updates, invites input on changes, and values shared decision-making. We work to build lasting relationships with parents, carers, and siblings, grounded in honesty, trust, and a shared goal: helping the person we support live a fulfilling life.
