Fairmont has a strong track record of supporting successful transitions from a variety of settings, including schools, colleges, family homes, hospital environments, and previous placements that have broken down.
We work in close partnership with families, placing authorities, and other providers to develop a tailored transition plan for each individual. No two plans are the same. Each one is thoughtfully designed to ensure a seamless and person-centred move, supporting both the individual and those important to them.
Clear communication and proactive planning from the outset have led to positive outcomes for the people we support and their families. For example, we supported one individual whose family visits at a previous placement had resulted in significant anxiety for both him and his parents. By introducing new routines and a carefully managed transition, we created space for positive family experiences. He now enjoys meaningful activities with his family, and visits consistently end on a positive note.
In another case, we supported an individual to move from their family home into a Fairmont residence. This was their first time living away from home. Understandably, this was a difficult decision for the family. However, with dedicated support for both the individual and their loved ones, the transition has been a great success. Moving into a residential setting has allowed the family to refocus their relationship on quality time and shared moments, rather than the challenges of daily care.
Fairmont has also successfully helped several individuals move from hospital settings back into the community, working in partnership with their Integrated Care Board (ICB) under the Transforming Care Programme. These transitions have been especially rewarding. Individuals are not only thriving in their new homes but are also actively engaged in their local communities.
